Kalkoup Community Care is a nurse-led mobile and clinic-based disability and complex care service located in West Perth, supporting children, adolescents, and young adults across Perth and surrounding areas.

 

Working alongside allied health professionals, our experienced nursing team provides comprehensive NDIS nursing services, in-home nursing care, clinic-based consultations, NDIS assessments, evidence reports, clinical care management plans, and specialised training for families and high-intensity support workers.

 

We bridge the gap between clinical recommendations and everyday life by supporting the implementation of feeding plans, enteral nutrition management, complex care routines, and safe clinical practices within the home and community setting.

 

Our focus is to ensure individuals receive consistent, high-quality care while empowering families and support teams with the knowledge, skills, and confidence required to manage complex health needs safely and effectively.
