Complex Community Care
Kalkoup Community Care is a nurse-led mobile and office-based service in Osborne Park, working alongside allied health professionals to support children and young adults with complex needs.
Our team provides in-clinic and in-home nursing care, NDIS assessments and evidence reports, clinical care management plans, and hands-on education for families and high intensity skilled support workers.
We help bridge the gap between clinical recommendations and what’s actually happening at home, supporting implementation of feeding plans, enteral care, and safe practice in the community.
By providing consistent clinical oversight, skill-building, and collaborative care, we aid in reducing hospital presentations and provide families the confidence to follow through on allied health plans safely and effectively.
