At Kalkoup Community Care, we’re redefining what community healthcare looks and feels like.
Based in West Perth, we are a nurse-led community nursing, disability support, and clinical consultancy service providing personalised care to children, adolescents, young adults, and families across Perth and regional Western Australia.
We are more than a service provider, and never an agency.
We are a dedicated team of experienced nurses, support workers, and allied health partners working together to deliver connected, compassionate, and clinically informed care that supports people to thrive at home, in the community, and throughout every stage of life.
Our mission is simple: to take the clinical edge out of healthcare and replace it with trust, collaboration, and genuine human connection.
Whether we’re providing in-home nursing care, NDIS support services, complex care management, clinical assessments, family education, or working alongside hospitals and allied health professionals, our focus remains the same, supporting better outcomes through prevention, early intervention, skill development, and long-term partnership.
At Kalkoup Community Care, we believe the best care happens when everyone works together.
We are stronger together.
Nurse-Led Clinical Expertise
Operating under the direction of Siobhan Hegarty, Kalkoup Community Care combines clinical excellence with practical, real-world support. Our team provides specialised nursing services, NDIS assessments and evidence reports, care coordination, complex disability support, enteral feeding support, high-intensity support worker training, and clinical consultancy services tailored to individual needs.
We bridge the gap between clinical recommendations and everyday implementation, helping families and support teams confidently manage complex health needs within the home and community environment.
Quality and Safety First
We are committed to delivering evidence-based, ethical, and best-practice care. Every assessment, recommendation, care plan, and service is provided with professionalism, transparency, and accountability, ensuring quality, safety, dignity, and person-centred care remain at the heart of everything we do.
Personalised and Culturally Responsive Care
No two individuals are the same. We take the time to understand each person’s goals, strengths, culture, preferences, and life experiences to ensure our support is genuinely tailored to their unique circumstances.
Our approach respects the whole person, not just their diagnosis or support needs.
Collaborative Care Partnerships
Exceptional outcomes are achieved through collaboration. We work closely with families, support coordinators, allied health professionals, educators, healthcare providers, and hospital teams to create seamless, coordinated care that aligns with each person’s goals and aspirations.
By combining clinical oversight, practical education, and ongoing support, we help build confidence, reduce unnecessary hospital presentations, improve continuity of care, and empower individuals to achieve greater independence and quality of life.
Supporting Perth Families and Beyond
From our West Perth location, we provide mobile nursing services, disability support, clinical consultancy, NDIS nursing assessments, complex care management, and family education across Perth and surrounding regions.
Our commitment is to deliver care that feels less clinical, more connected, and always centred around the people we serve.
